One such resident, E.S., a 27-year-old HIV-positive man from the Tel al-Hawa neighborhood of Gaza City, shared his heartbreaking story of survival.

E.S.'s Story

E.S. lives with his mother, a cancer survivor, and younger brother. The ongoing violence has made it dangerous for them to seek aid, and as E.S. noted, "People are fighting each other to get the aid boxes." His life has been significantly impacted as he is running out of medication, including essential antiretrovirals like tenofovir and lopinavir/ritonavir. "There are no more supplies coming in," he explains, indicating a dire shortage of necessary medications in the region. Though Israeli authorities claim no blockade on medicines, aid organizations report that crucial HIV treatments have been obstructed from entering Gaza.

The Consequences of Medication Shortage

The consequences of a lack of medication are dire. For E.S., who has limited mobility and relies on a walker, the risk to his health is substantial. Without his medication, his condition could deteriorate rapidly, posing a potentially lethal risk given the ongoing military hostilities. He fears being unable to walk altogether, especially with soldiers enforcing mass relocations and targeting civilians during evacuations. “I witnessed a family getting sniped right across the street,” E.S. recounted, highlighting the urban warfare unfolding around him.

A Difficult Choice

Despite the overwhelming fear, E.S. chose to remain in the northern part of Gaza, where the threat of violence was met with a semblance of stability compared to makeshift, unsafe shelters. "I saw people get sniped while trying to evacuate. Staying here, at least, feels a bit safer than living in a tent,” he added.

The Burden of Stigma

In a region where HIV stigma prevails, the burden E.S. carries is compounded by the fear of being outed. His family has yet to accept his status openly, and he worries that if they become aware of his medication struggles, it could endanger their safety. The stigma surrounding HIV exacerbates the emotional and psychological toll on those affected, especially in a culture where discussions of sexuality are taboo.

Rising HIV Infections in Gaza

According to reports, HIV infections in Palestine are increasing, largely due to lack of testing and healthcare services - especially in areas like Gaza, where war disrupts wellness protocols. A 2020 article highlighted that despite limited documentation, the risk of HIV infections is quite severe in the region.

E.S.'s Health Challenges

E.S. shared his background, vividly recalling his journey of self-discovery in relation to his sexuality and health. After spending time studying abroad, he returned to Gaza only to face additional health challenges, including untreated syphilis which escalated to neurosyphilis. His attempts to seek help were thwarted, creating a perfect storm where his health compounded without necessary care. “I felt trapped, unable to advocate for myself due to the taboo nature of my condition,” he reflected.

Survival Amidst Chaos

As the violence escalated in October 2023, E.S. found himself unexpectedly thrust back into chaotic circumstances reminiscent of moments from his past. The bombardments intensified, and he narrowly escaped death when a missile struck his home.

A Glimmer of Hope

In the midst of chaos, however, hope emerged. After much effort from healthcare professionals and organizations committed to assisting those in Gaza, a three-month supply of medications was secured for E.S. Despite the ever-present risk of supplies running out again and the grim situation on the ground exacerbating his health issues, E.S. expressed gratitude for the kindness shown to him, saying, "It's in the small mercies — acts of grace that keep us steadfast in Gaza."
